The Clerk of the Superior Court is a passport application acceptance facility where you can submit your U.S. Department of State DS‑11 passport application.
The U.S. Department of State offers online adult passport renewals, passport changes, and reporting of lost/stolen passports.
An appointment is required to submit a passport application.
Walk-ins will not be accepted.
Appointments are available Monday - Friday between 8:30 a.m. and 3:00 p.m., excluding court holidays.

- Each person applying for a passport, including minors, must be present at the passport appointment at our office.
- A Department of State DS-11 passport application form for each applicant, completely filled out in black ink. The Clerks Office currently accepts only DS-11 forms. (The Department of State may allow submitting a form to renew a passport directly to the Department State without going to a passport application acceptance facility. More information is available below in the "Where to Obtain Passport Forms and Other Information" section.)
- An original passport photo printed on matte or glossy paper to submit with your application.
- An original, physical, government issued photo identification document (Ex: Driver's License, US Military ID, Permanent Resident Card).
- If applying with a driver's license from outside the state of Arizona, you will need to present an original additional piece of photo identification (Ex: Costco card, Student ID, etc.).
- Proof of US citizenship (original birth certificate or naturalization certificate, old passport).
- A check or money order made out to the "Department of State" to pay the Department of State fees, including any optional Department of State expedited service fees. This payment must be separate from the Clerk of the Court processing fee and gets submitted with the application to the Department of State.
- A separate application processing fee of $35 for each application and any optional Clerk of Court expedited service fees payable to the Clerk of the Superior Court. Accepted forms of payment can be found at the Fee Schedule page.

Important Notice
A check or money order is required for payment to the U.S. Department Of State for your passport book/card and any optional expedited processing and shipping fees.
A separate form of payment is required for the Clerk of Court processing fee and optional Clerk of Court expedited shipping fee.
